POSITION SUMMARY - RN Case Manager


POSITION DUTIES - Utilization management, care coordination, discharge planning, revenue integrity


MINIMUM REQUIRED QUALIFICATIONS - NH/compact RN license

BLS

2 years recent acute hospital case management experience

Critical care experience

Must be familiar with inpatient case management, discharging, guardianship, surrogacy. Comfortable working with families and planning for LTC placement.


LENGTH OF ASSIGNMENT - 13 weeks


SHIFT / HOURS PER WEEK - M-F 8:00AM - 4:30PM with weekend rotation


SYSTEMS - Midas, Expanse & Careport


START DATE - 9/2/2025
